Never got the chance

13 yr old very congenial girl from Los Palos… thin and very dyspneic for a year… severe mitral stenosis on exam… signs of both right and left heart failure… huge heart on x-ray… admitted and started on just 20 mg of furosemide… somehow increasing dyspnea and tachycardia… O2 and propranolol 10 mg… worse yet… thinking of morphine and digoxin, maybe phlebotomy… never got the chance… another tribulation underdeveloped countries must endure.
